The Federal Emergency Management Agency (FEMA) has made minor revisions to the Standard Flood Hazard Determination Form and its accompanying instructions. Use of the revised form is mandatory as of July 1, 2006.
The only changes reflected in the new Standard Flood Hazard Determination Form are an updated Office of Management and Budget control number and an updated form expiration date. Minor changes were made to the instructions that accompany the form to reflect FEMA’s new contact information to be used for information requests when completing the form.
